Background:Anticholinergics are used for the treatment of conditions associated with a variety of chronic diseases and disorders. They are also used for the treatment of depression, smoking cessation and the control of other chronic conditions. However, the efficacy of these drugs for the treatment of depression and other related conditions has not been established.
Case report:A 32-year-old woman presented to the emergency department (ED) with a complaint of abdominal pain and a rapid heart rate (60 bpm). She had no history of substance abuse and had never had a seizure disorder. Her history included three medications taken for depression and smoking cessation, and two antidepressant drugs (SNRI and bupropion). Her primary care doctor prescribed an antidepressant for her.
She was prescribed bupropion, a 5mg oral tablet, for her current depressive episode. The medication worked well for her, and she was started on zyban. She continued to improve, and her depression resolved after the first week of treatment. She did not stop abruptly because of the adverse effects of zyban.
Aims:The objective of this study was to determine the effectiveness of an antidepressant in treating the depressive state of a patient with a history of substance abuse and psychiatric conditions.
Design:This was a case-control study.
Setting:Adult patients with major depressive disorder. Patients were randomly assigned to receive either bupropion (Zyban®) or a placebo, for a 7-week period.
Participants:A total of 7,000 adult patients with major depressive disorder, 4,000 of whom were randomized to bupropion (n = 7,000) or a placebo, for a 7-week period.
Main outcome measures:Anxiety, depression, and social anxiety disorder.
Results:There was no significant difference in the primary outcome measures between the bupropion and the placebo groups. Overall, the bupropion group had a higher incidence of anxiety, depression and social anxiety disorder than the placebo group.
Conclusions:Bupropion may be a useful treatment for patients with major depressive disorder.
